JUST A NOTE.....

IF YOUR CHILD REQUIRES ANY KIND OF MEDICATION THROUGHOUT THE DAY, IT NEEDS TO BE BROUGHT INTO THE CLINIC BY A PARENT/GUARDIAN. THE PROPER PAPERWORK NEEDS TO BE SIGNED BY THE PARENT AND/OR PHYSICIAN IN ORDER FOR MEDICATIONS TO BE GIVEN AT SCHOOL.
OVER-THE-COUNTER MEDICATIONS (SUCH AS ADVIL, TYLENOL, COLD MEDICINE) REQUIRE ONLY A PARENT SIGNATURE ON A SCHOOL MEDICATION AUTHORIZATION FORM.
FOR PRESCRIPTION, HOMEOPATHIC OR SUPPLEMENTAL MEDICATIONS A LICENSED GEORGIA PHYSICIAN MUST ALSO SIGN THE SCHOOL MEDICATION AUTHORIZATION OR HEALTHCARE PLAN ALONG WITH THE PARENT.
*ALL MEDICATIONS MUST BE IN THEIR ORIGINAL CONTAINER WITH UNEXPIRED DATES AND LABELED IN ENGLISH.
*STUDENTS CANNOT BRING MEDICATION BACK AND FORTH FROM HOME TO SCHOOL. MEDICATIONS NEED TO BE BROUGHT IN AND PICKED UP BY AN ADULT.
